How Do I...
To book an appointment at the library, fill out our Passport Appointment Request form. An agent will be in touch with you to confirm the best day and time for your appointment within three business days. Please note that appointment hours are held weekly on Tuesday and Thursday evenings, and once a month on Monday afternoons. We typically book three to four weeks in advance.
If these days do not work for you, there are other Acceptance Facilities nearby with additional days and times!
You MUST apply in person using Form DS-11 if you respond "Yes" to at least one of the following statements:
- I am applying for my first U.S. passport.
- I am applying with my child who is under age 16.
- My previous U.S. passport was issued when I was under age 16.
- My previous U.S. passport was lost, stolen, or damaged.
- My previous U.S. passport was issued more than 15 years ago.
Renew by mail using Form DS-82 if you respond "Yes" to all statements below about your most recent passport:
- I can submit it with my application.
- It is not damaged (other than normal wear and tear).
- I have never reported it lost or stolen.
- It was issued within the last 15 years.
- It was issued when I was age 16 or older.
- It was issued in my current name, or I can provide another document such as a marriage certificate or divorce decree that is evidence of my name change.
- It was not limited to less than the normal 10 year period of validity for an adult passport due to multiple passport thefts or losses, or due to issues with passport damage or mutilation.
Passport Photos MUST be to the standards set by the US Department of State in order for your Passport Application to be accepted.
- Submit one color photo, taken in last 6 months.
- Use a clear image of your face. Do not use filters commonly used on social media.
- Have someone else take your photo. No selfies.
- Take off your eyeglasses for your photo.
- Use a white or off-white background without shadows, texture, or lines.

You can pay to get a passport photo printed at several places. Before arriving to get your photo taken, we recommend calling the location to make sure their passport photo service is available.

There are general forms and items that every applicant MUST have with them to submit an application:
- DS-11 Form
- Form should be completed in black ink
- DO NOT sign the form, the form must be signed in the presence of an acceptance agent
- Evidence of US Citizenship
- Full validity, undamaged U.S. passport (expired passports are accepted)
- Full validity means the document is/was valid for 10 years for adults and 5 years for children under 16.
- U.S. birth certificate that meets the following requirements:
- Issued by the city, county, or state of birth
- Lists your full name, date of birth, and place of birth
- Lists your parent(s)' full names
- Has the date filed with registrar's office (must be within one year of birth)
- Has the registrar's signature
- Has the seal of the issuing authority
- Consular Report of Birth Abroad or Certification of Birth
- Certificate of Naturalization
- Certificate of Citizenship
- Full validity, undamaged U.S. passport (expired passports are accepted)
- You must present one of the following identification documents (IDs) to the acceptance agent
- Valid or expired, undamaged U.S. passport book or passport card
- In-state, fully valid driver's license or enhanced driver's license with photo
- Certificate of Naturalization
- Certificate of Citizenship
- Government employee ID (city, county, state, or federal)
- U.S. military or military dependent ID
- Current (valid) foreign passport
- Matricula Consular (Mexican Consular ID) - commonly used by a parent of a U.S. citizen child applicant
- U.S. Permanent Resident Card (Green Card) - commonly used by a parent of a U.S. citizen child applicant
- Trusted Traveler IDs (including valid Global Entry, FAST, SENTRI, and NEXUS cards)
- Enhanced Tribal Cards and Native American tribal photo IDs
- Note: you may be asked to present an additional ID when presenting one of these three documents: 1) In-state, fully-valid learner's permit with photo, 2) In-state, fully-valid non-driver ID with photo, or 3) temporary driver's license with photo.
The state offers a Passport Fee Calculator as well as a Passport Fee Chart for reference. During your appointment, the agent will be able to walk you through the required fees, as well as optional fees for services such as expedited shipping.
There are two different fees that need to be paid:
- Fee to the Department of State: MUST be in the form of a check or a money order
- Fee to the Acceptance Facility (Library): Is $35 per application, and can be paid in cash, card or check

Passports for children under the age of 16 have a few different requirements to be aware of.
- All children under the age of 16 MUST apply in person and ALL parents MUST be present
- You must submit documentation that lists the parent(s) or legal guardian(s) of the child applying for a passport. Examples include:
- U.S. birth certificate (also evidence of U.S. citizenship)
- Foreign birth certificate
- Adoption decree
- Divorce/Custody decree
- All parents or guardians MUST present one form of ID
- All parents or guardians MUST give consent for the passport to be issued

Receiving Passport and Supporting Documents
Once your passport is printed, the passport and supporting documents are mailed back depending what you applied for:
Passport Book: You may receive your newly-issued passport and your citizenship documents in two separate mailings. You may wait 8 weeks after receiving your passport before you receive a second mailing with your citizenship documents.
Passport Card: You may receive your newly-issued passport card and your citizenship documents in two separate mailings. You may wait 8 weeks after receiving your passport before you receive a second mailing with your citizenship documents.
Both Passport Book and Card: You may receive three separate mailings; one with your citizenship documents, one with your newly-issued passport book, and one with your newly-issued passport card. You may wait 8 weeks after receiving your passport book before you receive a second and third mailing with your passport card and citizenship documents.
It may take 8 weeks after you receive your new passport for your supporting documents (e.g. birth certificate or previous passport) to arrive. If you have been waiting more than 8 weeks, contact the National Passport Information Center at 1-877-487-2778 to report that you have not received your documents.
